Eden Health - - Responsibilities: Deliver in-home physical therapy modalities appropriate to the patient's clinical status and needs; in accordance with the standard of care; Review caseload with supervising PT on a regular basis as established by best practice and/or State guidelines; Complete accurate and thorough visit notes via Electronic Health Record (EHR) on every patient in a timely fashion; Educate patients, their families/caregivers, and other staff as appropriate, in physical therapy modalities; Identify risk of injuries or threats to life and health in the patients environment and implement appropriate interventions to reduce those risks
