Eden Health - - Responsibilities: Implement treatment plans ordered by the physician as indicated by the plan of care to all patients on caseload; Review caseload with supervising PT on a regular basis as established by best practice and/or State guidelines; Educates patients and families in appropriate physical therapy modalities; Delivers physical therapy modalities appropriate to the patient's clinical status and needs; Completes accurate and thorough visit notes via Electronic Health Record on every patient
